As part of a post-graduate research study for Maryland Institute College of Art (MICA) ‘s User Experience (UX) Design program, this survey will help in the selection of participants for interviews which will inform design research for a thesis project.
If selected for an interview you will receive your choice of $25.00 (U.S.) Amazon or Starbucks gift card as a thank you for participating. Please do not provide any PHI or HIPAA-protected information on this survey or during the interview, if applicable. *
